Personal information is collected, used and disclosed in connection with the Vital Statistics Registry in accordance with the provisions of
The Vital Statistics Administration Transfer Act,
The Vital Statistics Act, 2009 and
The Change of Name Act, 1995 (as applicable and as such legislation may be amended or replaced from time to time).
Collection
Examples of sources of personal information for the Vital Statistics Registry include the following:
- Individuals provide information about themselves.
- Officials conducting marriage ceremonies provide information for marriage registrations.
- Physicians and coroners provide information for medical certificates of death.
- Saskatchewan Regional Health Authorities provide information for birth, stillbirth and death registrations.
- ISC receives registration information from Saskatchewan Ministry of Health (as defined in The Health Information Protection Act) for use in connection with the registration of births, stillbirths, marriages or deaths, or the issuance of certificates of birth, marriage or death.
Use
Examples of uses of personal information in the Vital Statistics Registry include the following:
- Maintenance and administration of the Vital Statistics Registry.
- Compiling, publishing and distributing statistical information.
Access and disclosure
The Vital Statistics Registry is a private registry. Members of the public are not permitted to access or obtain information within the registry other than information about themselves or as permitted by law and authorized by the Registrar. In limited situations and as authorized by law, certain agencies are also able to access information from the registry. Below are examples of who may access personal information contained in the registry. Please note that the information discussed below is general in nature and the relevant legislation and regulations provide specific details with respect to the requirements necessary to access such personal information.
Registration of birth
A certificate, a certified copy or photographic print of the registration of a birth other than the registration of birth of a person who is the subject of an adoption, may be issued to the following persons:
- the person who is the subject of the birth registration.
- a parent or legal custodian of the person who is the subject of the birth registration.
- a personal or property guardian, or the legally appointed representative, of the person who is the subject of the birth registration.
Registration of marriage
A certificate, certified copy or photographic print of the registration of a marriage may be issued to:
- a person who is the subject of the marriage registration.
- a child of the marriage where the document is required.
- prove parentage, citizenship, or for other legal purposes.
Registration of death or stillbirth
A certificate, certified copy or photographic print of the registration of a death or stillbirth may be issued to:
- the spouse, an adult child, an adult sibling (stillbirth registration) or a parent of the deceased.
- the legal representative of the deceased, when required for any purpose related to the administration of the estate of the deceased.
- any person responsible for the deceased person's body, when required for disinterment purposes or for transportation of the body to another province or country.
- a physician who requires the information for purposes related to the provision of treatment to a member of the deceased person's family.
Agencies
Examples of agencies that receive personal information from the Vital Statistics Registry include the following:
- The Saskatchewan Ministry of Health is authorized to receive information from the Vital Statistics Registry for any purpose related to the mandate of the Ministry (such as maintaining the accuracy of information in the provincial health registry).
- Statistics Canada receives information for the purposes of national statistics.
Information is provided to federal and provincial government agencies for the purposes of maintaining the accuracy of their registries.
- RCMP and provincial/municipal police services.
- Except where authorized by law, all disclosures from the Vital Statistics Registry must be reviewed and approved by the Vital Statistics Registrar.
