Order a Birth Certificate
A birth certificate is your most valuable identity document. It's a legal document issued by the Province.
You need a birth certificate to get other important identification documents, like a:
- Driver's license
- Passport, or
- Social Insurance Number

Step 1:
Choose the type of Birth Certificate
Short Form | ($25) | See example
Contains the individual's full name, date of birth, place of birth, sex, registration number and registration date.
Short-form certificates may not be ideal for children ages 16 and under as many government agencies require the additional information contained on the long-form certificate.
Long Form | ($25) | See example
Contains the same info as the short form, plus the parents’ full names and their province/state and country of birth.
Long-form certificates are generally preferred by government agencies - like Passport Canada and Indian and Northern Affairs - for children ages 16 and under.
Genealogical photocopy ($50)
Contains all of the information that appears on the original birth registration and is stamped "FOR GENEALOGY ONLY".
Certified photocopy ($50)
Contains all of the information that appears on the original birth registration.

As an applicant, you must submit:
- A copy of one piece of government-issued photo identification, or
- Copies of two other pieces of identification - one must have your signature on it
