Order a Birth Certificate 

You can order a birth certificate for:

  • Yourself
  • Your children
  • People for whom you are legal guardian
  • A person who has given you written authorization
  • A deceased relative

Step 1: Choose the type of Birth Certificate 

Short Form ($25) | See example
(Contains the individual's full name, date of birth, place of birth, sex, registration number and registration date)

Long Form ($25) | See example
(Contains the same info as the short form, plus the parents’ full names and their province/state and country of birth)

Genealogical photocopy ($50)
(A photocopy of the original birth registration for genealogical purposes)

Certified photocopy ($50)
(A photocopy of the original birth registration)

Step 3: Fax or mail the completed form with payment
