Order a Death Certificate 

A death certificate is required:

  • As legal proof of the death
  • To settle an estate
  • To receive entitlements for benefits such as pensions and insurance

 

Who can apply for a Death Certificate

You can order a death certificate if you are:

  • The spouse, parent or child of the deceased person 
  • The executor or administrator of the deceased person's estate
  • A person who has written authorization from the spouse, parent or child of the deceased person, or from the executor of the deceased person's estate
  • A person who needs a death certificate for use in a court of law, for settlement of the deceased person's estate or for disinterment purposes

Step 1: Choose the type of Death Certificate 

Frame Certificate ($25) | See example
Contains the full name of the deceased, date of death, place of death, age, sex, marital status, registration number and registration date.

Genealogical photocopy ($50)
Contains all of the information that appears on the original death registration excluding cause of death and is stamped "FOR GENEALOGY ONLY".

Certified photocopy ($50)
Contains all of the information that appears on the original death registration excluding cause of death.

Step 2: Complete the Death Certificate Order Form (PDF)

As an applicant, you must submit:
  • A copy of one piece of government-issued photo identification, or
  • Copies of two other pieces of identification - one must have your signature on it
