Order a Birth Certificate 

A birth certificate is your most valuable identity document. It's a legal document issued by the Province.

You need a birth certificate to get other important identification documents, like a:

  • Driver's license
  • Passport, or
  • Social Insurance Number

 

Who can apply for a Birth Certificate

You can order a birth certificate if you are an individual ordering:

  • Your own certificate
  • Your child's certificate - as long as you are:
    • Recorded as a parent on that child's birth registration 
    • A legal guardian of the child (guardianship documents must be provided), or 
    • A custodian of the child (custody documents must be provided)
  • Someone else's certificate (like your current spouse) - but only with that person's written authorization
  • Your deceased spouse's certificate

Step 1: Choose the type of Birth Certificate 

Short Form | ($25) | See example
Contains the individual's full name, date of birth, place of birth, sex, registration number and registration date.

Short-form certificates may not be ideal for children ages 16 and under as many government agencies require the additional information contained on the long-form certificate.

Long Form | ($25) | See example
Contains the same info as the short form, plus the parents’ full names and their province/state and country of birth.

Long-form certificates are generally preferred by government agencies - like Passport Canada and Indian and Northern Affairs - for children ages 16 and under.

Genealogical photocopy ($50)
Contains all of the information that appears on the original birth registration and is stamped "FOR GENEALOGY ONLY".

Certified photocopy ($50)
Contains all of the information that appears on the original birth registration.

Step 2: Complete the Application for Birth Certificate (PDF)

As an applicant, you must submit:
  • A copy of one piece of government-issued photo identification, or
  • Copies of two other pieces of identification - one must have your signature on it
