Corporate Entity Change of Name - Instructions

Use this form when a corporation’s legal name has changed.

Note: When a Corporation changes its name through the Corporate Registry, the COBRA linked client number and any client numbers branched to the COBRA linked client number will automatically be changed in the Land Registry. Any other client numbers (convenience, external, etc.) will not be automatically updated.  Convenience numbers were assigned at the time of conversion to an electronic record keeping system (2000-2003).

Note: The Corporate Entity Client Number Amalgamation form can be submitted at the same time as the Corporate Entity Change of Name form.

SECTION A – CLIENT INFORMATION
 
Enter the New Corporate Name as it should appear in the Land Registry.

SECTION B – OTHER CLIENT INFORMATION
Enter the Client Number(s) and the Existing Corporation Name that corresponds with that Client Number exactly as it currently exists in the Land Registry.

If the Client Name and Client Number do not match those in the Land Registry, the application will be rejected.

List all the Client Numbers and Existing Corporate Names that you wish to change to the New Corporate Came indicated in Section A.

If there is not enough space in the boxes provided or if you wish to provide the Client Number(s) and Corporate Name associated with those numbers on a separate list please indicate in the Attached check box and attach the list to the form.

SECTION C- CONTACT INFORMATION
Provide the contact information for the person to contact if any questions arise in reviewing the application or to inform that the request has been completed.

Affidavit for Corporate Entity Change of Name

REQUIRED ATTACHMENTS TO APPLICATION
Please complete and attach the Affidavit for Corporate Entity Change of Name as required under section 84 of The Land Titles Regulations, 2001 with supporting documentation (such as Certificate of Amalgamation or Certificate of Amendment for Corporate Name Change).

INFORMATION REGARDING PERSON MAKING APPLICATION
An authorized officer for the entity must indicate their Name and the Town/City and Province where they live.

PARAGRAPH 1 – CAPACITY OF PERSON MAKING APPLICATION
The applicant must indicate their Title for the entity and indicate the Corporation Name as it currently exists within the Land Titles Registry.

PARAGRAPH 2 – NEW NAME
Must specify the New Legal Name for the entity.

SIGNATURE AND JURAT
The applicant must swear or affirm to the truthfulness of the affidavit material.
The applicant must sign on the Client Signature line.

In the Jurat section, completed by the oath taker, indicate the Place, Province and the Date (day, month year), where and when the Affidavit was sworn or affirmed.

Have a Commissioner for Oaths for Saskatchewan or Notary Public or Solicitor sign on the line.

Enter commission /appointment expiry date of the Commissioner/Notary Public.  If there is no expiry date or the oath is taken by a Commissioner/Notary Public who holds the position by way of profession, please indicate the profession (i.e. police officer, solicitor, etc.).

If the Affidavit is sworn or affirmed outside Saskatchewan by an official under their official seal, ensure that the seal is clearly visible when submitted to ISC. For information on who may commission an Affidavit outside of Saskatchewan, please see section 24 of The Land Titles Regulations, 2001, available from Queen’s Printer, B19-3085 Albert St., Regina, SK S4S 0B1 or www.qp.gov.sk.ca.

Note: If the witness wishes to affirm, please make the appropriate changes to the Affidavit.
