Use this form when a corporation wishes to amalgamate several client numbers in the Land Registry to one client number.

When a Client Number is amalgamated, it is “absorbed” by the Client Number it is amalgamated to, and ceases to exist. The address and delivery preference will be that of the remaining Client Number.  All Client Numbers being amalgamated must be held by exactly the same name.

Note: If submitting a Corporate Entity Change of Name or Corporate Entity Name Correction form at the same time as the Corporate Entity Client Number Amalgamation form, you must use the new name.

SECTION A – CLIENT INFORMATION
Enter the Client Number which you want to amalgamate all the other Client Numbers to and the Corporate Name exactly as it currently is in the Land Registry.

Make sure that the Client Number corresponds with the Corporate Name or the application will be rejected.

Note: Amalgamating Corporate Client Numbers

• A COBRA linked Client Number indicates that the corporation is an active, registered corporation with the Saskatchewan Corporate Registry.
• A COBRA linked Client Number cannot be amalgamated to another COBRA linked Client Number. Amalgamating Client Numbers does not amalgamate corporations. Amalgamation of corporations occurs through the Corporate Registry.
• A COBRA linked Client Number cannot be amalgamated to a convenience Client Number.
• A convenience Client Number can be amalgamated to a COBRA linked Client Number or another convenience Client Number.

SECTION B – OTHER CLIENT NUMBERS
Enter the Client Number(s) that you want to amalgamate to the Client Number in Section A. Enter the Corporate Name(s) that correspond with the Client Number(s) exactly as it currently exists in the Land Registry.

If the Client Name and Client Number do not match those in the Land Registry, the application will be rejected.

If there is not enough space in the boxes provided or if you wish to provide the Client Number(s) and Corporate Name associated with those numbers on a separate list please indicate in the Attached check box and attach the list to the form.

SECTION C- CONTACT INFORMATION
Provide the contact information for the person to contact if any questions arise in reviewing the application or to inform that the request has been completed.

Affidavit for Corporate Entity Client Number Amalgamation

Please make sure the Affidavit for Corporate Entity Client Number Amalgamation is attached.

An authorized officer for the entity must indicate their Name and the Town/City and Province where they live.

PARAGRAPH 1 – CAPACITY OF PERSON MAKING APPLICATION
The applicant must indicate the Corporation Name they are an authorized officer of the corporation.

SIGNATURE AND JURAT
The applicant must make an oath saying that he/she is an authorized signing officer for the corporation (1) in Section A, that the clients in Section B of the application are the same as the client in Section A.

The applicant must sign on the Client Signature line.

In the Jurat section, completed by the oath taker, indicate the Place, Province and the Date (day, month year), where and when the Affidavit was sworn or affirmed.

Have a Commissioner for Oaths for Saskatchewan or Notary Public or Solicitor sign on the line.

Enter commission /appointment expiry date of the Commissioner/Notary Public.  If there is no expiry date or the oath is taken by a Commissioner/Notary Public who holds the position by way of profession, please indicate the profession (i.e. police officer, solicitor, etc.).

If the Affidavit is sworn or affirmed outside Saskatchewan by an official under their official seal, ensure that the seal is clearly visible when submitted to ISC.
